You agree you have fully read, understood, and made sure you have asked questions and had all of your questions answered to your complete satisfaction before scheduling an appointment with us.
If you do not agree with any of these policies, please do not schedule a massage session with Vitality Health Massage.
1. Cancellations & Rescheduling
48-Hour Cancellation Policy
Cancellations or reschedules must be made at least 48 hours before your scheduled appointment.
Appointments canceled with less than 48 hours' notice, as well as missed appointments (no-shows), will be charged the full session fee.
The missed appointment fee may be charged to the card on file or deducted from an available membership session, package, or account credit.
If the missed appointment fee is not paid, all future scheduled appointments will be canceled, and any active membership will be placed on hold until the outstanding balance has been paid in full.
Rescheduling
If online booking shows limited availability, please contact us directly. We may be able to accommodate your scheduling needs.
Always keep your appointment confirmation and any cancellation or rescheduling confirmation emails as proof of changes made to your appointment.
Appointment Reminders
Appointment reminders are sent as a courtesy. It is the client's responsibility to know and keep track of their scheduled appointment time.
Failure to receive a reminder by email or text does not waive the cancellation policy or missed appointment fee.
2. No-Shows
A no-show means failure to cancel or reschedule within 48 hours or not showing up.
No-shows will be charged the full price of the session.
All outstanding charges must be paid before booking another session.
3. Late Arrivals
Sessions may be shortened if you arrive late.
More than 15 minutes late means the full session charge, and your therapist reserves the right to cancel your appointment.
4. Sickness Policy
Do not attend your appointment if you are sick or contagious—examples include flu, fever, diarrhea, vomiting, skin infections, and lice.
Cancellation fees apply if notice is less than 48 hours.
A doctor’s note may be required before rescheduling in certain cases.
The therapist reserves the right to cancel or refuse service if illness may cause harm.
5. Payment
All payments for sessions, packages, memberships, add-ons, and events are final with no refunds.
We accept Square (Visa, MasterCard, Discover, AmEx) and cash.
Please review our policies regarding card storage, security measures, payment information, scheduling responsibilities, and privacy.
6. Purpose of Massage & Scope of Practice
Massage is for stress reduction, relaxation, pain relief, circulation, and mobility—not a substitute for medical care.
Massage therapists do not diagnose, prescribe, or perform spinal or skeletal adjustments.
Always inform your therapist of health changes.
7. Privacy
All session details remain confidential. Information is only shared with written consent or if required by law.
8. Professional Boundaries & Conduct
Massage is therapeutic and non-sexual.
Inappropriate behavior will result in immediate termination of the session, full charge, and permanent dismissal from the practice.
Draping is required at all times. Only the area being treated will be undraped, and client comfort is always respected.
9. Clients Under 18
Parent or guardian must sign the intake form, schedule the appointment, be present at the first session, and remain in the room for all treatments.
10. Gift Certificates
Expire after 2 years.
Non-refundable, have no cash value, and are non-transferable without written consent.
Must be presented at time of service.
Cannot be split into multiple sessions.
If rates increase, the client pays the balance.
11. Memberships
All sales are final; no refunds.
May be paused or canceled twice during the life of the membership.
Unused sessions expire after 6 months once a membership is paused or canceled.
Rollover sessions expire by the end of the following month.
You may substitute your appointment with 48-hour notice. The substitute must complete an intake form, including full name, telephone number, and email address.
The massage therapist reserves the right to cancel a membership for policy violations.
12. Packages & Credits
Packages typically include 3 sessions at a discounted rate.
They expire 2 years from purchase and are non-refundable. As of 9/1/2026, packages will expire after 6 months.
Credits are non-transferable, non-refundable, and may be revoked for policy violations.

14. Additional Policies

Smoke-Free: Avoid smoking before your session to protect sensitive clients.
Arrival: Please arrive 10 minutes early with forms completed. Sessions will end on time even if paperwork delays your start.
15. Session Length
Appointment times are reserved specifically for each client based on the session length originally scheduled.
Any request to shorten, lengthen, reschedule, or otherwise modify an appointment must be made at least 48 hours before the scheduled appointment time.
If a longer session is reduced with less than 48 hours' notice, the client remains responsible for the full cost of the originally scheduled session.
Requests to extend a session must be made at least 48 hours in advance and may be accommodated only if sufficient time is available and the therapist approves.
Approval is not guaranteed. Additional fees apply for approved increases in session length.
By scheduling an appointment, clients acknowledge and agree to this appointment modification policy.
Booking for Another Person – Payment Authorization Policy
If you schedule an appointment for another person, you agree to be financially responsible for the appointment and authorize the card on file to be charged for applicable appointment, cancellation, no-show, or other policy fees. If you do not agree, please do not schedule on another person's behalf.
General Policies
16. Specialty Service Policies: Cupping Therapy & Hot Stone Massage
Informed Consent
Cupping therapy and hot stone massage are optional enhancements. By scheduling or accepting either service, you acknowledge that you have read and understand the information below, had the opportunity to ask questions, and voluntarily consent to treatment.
These services are not intended to diagnose, treat, cure, or prevent any medical condition and are not a substitute for medical care.
Cupping Therapy
Cupping uses specialized cups to create suction on the skin, which may help reduce muscle tension, improve circulation, and support recovery.
Possible Risks and Side Effects
Temporary circular marks or discoloration, bruising, skin redness, mild swelling, tenderness, soreness, temporary tightness or sensitivity, lightheadedness, dizziness, fatigue, temporary discomfort, and rarely blistering, skin irritation, or minor skin injury.
Cupping Contraindications
Notify your therapist before treatment if you have pregnancy, a bleeding or clotting disorder, use blood-thinning medications, diabetes with impaired healing, cancer or current cancer treatment, fragile or easily bruised skin, varicose veins, active skin infections or rashes, eczema or psoriasis flare-ups, sunburn, open wounds, recent surgery, recent fractures or acute injuries, fever or contagious illness, or any condition affecting skin integrity or healing.
Hot Stone Massage
Hot stone massage uses professionally heated stones to help relax muscles and enhance therapeutic massage.
Possible Risks and Side Effects
Temporary skin redness, mild soreness or tenderness, increased sensitivity to heat, bruising, lightheadedness, fatigue, and rarely minor burns or skin irritation.
Hot Stone Contraindications
Notify your therapist before treatment if you have reduced sensation or neuropathy, diabetes affecting circulation or sensation, cardiovascular disease, poor circulation, uncontrolled blood pressure, multiple sclerosis or another neurological disorder affecting heat sensitivity, active skin conditions, sunburn, open wounds, recent surgery, acute inflammation, fever or contagious illness, pregnancy, or any condition affecting your ability to safely tolerate heat.
If stones feel too warm or uncomfortable, notify your therapist immediately.
Client Responsibility
You agree to provide complete and accurate information regarding health history, medications, allergies, injuries, surgeries, pregnancy status, and medical condition changes before each appointment.
Failure to disclose relevant information may increase risk and require modification or discontinuation of treatment.
Your therapist may decline or modify treatment if it is not appropriate or may pose a risk to your health or safety.
Assumption of Risk and Release
By accepting cupping or hot stone massage, you acknowledge the information, nature, benefits, and known risks; had the opportunity to ask questions; understand there are no guarantees; voluntarily assume ordinary and inherent risks; agree to report pain, excessive discomfort, unusual heat, dizziness, or unexpected reactions; and release and hold harmless the therapist and practice for ordinary risks and expected side effects when treatment follows accepted professional standards, except where prohibited by law or involving negligence or willful misconduct.
17. Health Information and Client Responsibility
Clients must provide complete and accurate health information on the intake form, including relevant medical conditions, allergies, medications, injuries, surgeries, pregnancy status, and any changes before each appointment. Clients are responsible for informing their therapist of any new or changed health information so treatment can be reviewed, modified, or declined when needed.
18. Emergency Procedures
For a serious medical emergency, such as a heart attack, stroke, choking, or seizure, 911 will be called immediately. For other urgent situations, the emergency contact listed on the client's intake form may be contacted. Clients are responsible for keeping their emergency contact information current.
19. Client Records
Client records, including intake and health information, will be retained for 10 years, subject to applicable law and professional requirements. Clients should keep their contact and health information current.
20. Appointment Reminder Consent
When clients schedule an appointment, they agree to receive appointment reminders by text message, phone, and email unless they tell their therapist how they prefer to be contacted. Appointment reminders are a courtesy and do not replace the client's responsibility to track appointment times.
21. Payment Disputes
Clients agree to contact Vitality Health Massage first to discuss and resolve payment concerns before filing a chargeback or payment dispute, when permitted by applicable law. This does not limit any rights provided by law.
